To file a City Claim Form for personal injury or property damage, please visit the City Clerk's webpage.  Download and fill out the form and submit to the City Clerk's Office at 1315 Valley Drive, Room 203, Hermosa Beach, CA  90254.  The claim will be placed on a City Council agenda for action and potential referral to the City's Liability Claims Administrator for resolution.
